Transferring Care In

If you'd like to transfer your ongoing ADHD monitoring and treatment to us, we're happy to welcome you to the Attforde ADHD Centre.

Most providers will issue you with an ADHD report summarising your original assessment, usually based on the DIVA-5 structure. We ask for a copy of this report before your transfer of care appointment, so please have it ready when you book.

If you don't have a copy but still want to transfer your care, email us at queries@adhdcardiff.co.uk and we'll advise on next steps.

Transferring Care Out

We recognise that for many people, Attforde ADHD Centre is a stop-gap between private assessment and ongoing care with the NHS.

If a diagnosis is made, you'll be given a report outlining the diagnosis against the DIVA-5 criteria. This report can be passed on to your new ADHD provider to support your ongoing care.

Should you require an additional copy of your original diagnosis report, this is available for a flat £10 administration fee — email us at queries@adhdcardiff.co.uk to request one.
